FOR NEW PATIENTS SEEKING AN APPOINTMENT
Please go to Request a Clinic Appointment for advice on where to send the referral letter. Once we have received the referral letter we will contact you to agree a date. Please ask the referrer to include your contact telephone number and your email address.
After reviewing the referral letter we will contact you to agree a date, and then send you a letter of confirmation with further information about the appointment.
Please note that due to high demand the waiting time is approximately 16-18 weeks. Cancellations are very rare. As such it is not possible to see you urgently or at short notice.
FOR PATIENTS SEEKING A FOLLOW-UP, OR TO MAKE A GENERAL ENQUIRY
Alternatively, if you have a general enquiry please go to Send an Enquiry and complete the form. If you are a known patient of Dr Hakim’s seeking a follow up you will already have the contact details to email Dr Hakim.
PREPARING FOR YOUR CLINIC APPOINTMENT
Dr Hakim encourages you to bring your own notes about your medical history and your family history; also, if available the results of any tests and copy of pertinent letters etc., from any one who has assessed you.
